Advance Directive 2-Hour Workshop: Let’s Finish Your Advance Directive Together

A relaxed follow-up session for those who have already started their Advance Directive.
The focus will be on helping you finalize and sign your document. No presentation; facilitators will be available to work with you to answer your questions. Bring your partially completed form and your questions.
If you complete your Advance Directive at this session, facilitators will be able to serve as witnesses upon your signature. We also plan to have scanning capabilities to email you a PDF copy of your completed, signed and witnessed form to distribute.
Free community workshops designed to help you complete a Short Form Advance Directive (VT Ethics Network) with clarity, confidence, and support. Each workshop is 2 hours in length.
