FLU & COMMUNITY HEALTH NURSE COORDINATOR – Part-time position 20-30 hours per week – housed in Rutland but services all locations

The VNA & Hospice of the Southwest Region is looking for a dynamic registered nurse to coordinate the agency’s flu and community health programs throughout Rutland and Bennington Counties.

The primary responsibility of the Flu & Community Health Nurse Coordinator is to organize, setup, run, and breakdown various clinics held in public settings.  This position may work alone or direct a small team of clinicians and volunteers.  The position is responsible for ensuring that all clinics follow internal policies, HIPAA regulations, and Universal/Standard Precautions.  This position is responsible for controlling clinic inventory and timely return of all documentation and supplies.

Qualifications:

  • Possession of valid RN Vermont State Registered Nurse license by date of hire
  • Valid driver’s license
  • Must be organized, detail oriented, able to work within an established budget

BEHAVIORAL HEALTH NURSE/ HOME CARE NURSE – Part-time 30 Hours – Bennington office

We are looking for a behavioral health nurse to work in our homecare program.

Key responsibilities:
  • Carry out behavioral health assessments on clients in their homes
  • Collaborate with team members on the coordination of care.
  • Prepare and deliver in-service trainings to co-workers on behavioral health issues related to coordination of care.
 Qualifications:
  • Valid Vermont nursing license required.
  • Valid State Driver’s license required.
  • Prior behavioral health nursing experience highly desired.

We are looking for registered nurses to work in our home care program. As a home health nurse, you will be part of a skilled and motivated team of nurses and therapists providing care to clients in their homes.

Key responsibilities:

  • Serve as RN-nurse case manager for primary caseload
  • Collaborate with nursing, therapy, and other clinical staff to provide patient care

We’re seeking compassionate, team-oriented individuals to provide a full range of professional nursing services to clients of all ages including admission, evaluation, and care plan development. Working with a team of home care professionals and paraprofessionals, our nurses strive to achieve the highest level of coordinated care for clients served in Rutland County, Dorset, and Rupert.
