Registered Nurse – Nurse Family Partnership – Agency Wide

Part time (up to 30 hours per week)

We are looking for a registered nurse to work in our Children and Families program as part of the Nurse Family Partnership (NFP) program.

The NFP is a pre-natal, post-partum nursing program that focuses on first time, low income families.

Key responsibilities:

  • Provide new client intake meetings for potential clients
  • Build up-to a 25 client caseload
  • Provide life-skill, parental, child-care coaching for pending and new moms
  • Follow NFP guidelines and regulations for documentation of interventions
  • Familiarize self with NFP requirements

  • BSN degree required.

Clinical Manager – Full Time – Rutland Office

We are looking for a full-time hospice clinical manager to supervise and oversee the hospice nursing team.

Key Responsibilities:

  • Oversight, management, and supervision of hospice nursing staff.
  • Serve as a resource to hospice patients, staff, and others as needed.
  • Provide direct patient care as needed to meet demands of the program.
  • Provide information and proactively participate in Interdisciplinary Team (IDT) meetings.

Requirements:

  • Valid VT State Nursing license required.
  • Previous management experience highly desired

We are looking for a full time registered nurse to work in our Hospice Program.

Certified Diabetes Educator, Registered Nurse – Rutland Office

The VNAHSR is looking for a Certified Diabetes Nurse Educator to work in our home care program.

Key Responsibilities:

  • Provide diabetes education and nursing intervention to adult and pediatric clients in their homes
  • Develop and educate patients in self-management plans
  • Be a resource for peers on disease management, symptoms, etc
  • Reconcile medications and make recommendations to physician as needed

Requirements:

  • Current Vermont RN license
  • CDE certification or previous diabetes education experience so as to qualify for CDE exam
