RAVNAH’s Case Managers and the Choices for Care Program Keep Patients Safe and Independent in the Comfort of Home
WHAT IS THE CHOICES FOR CARE PROGRAM?
Choices for Care provides personal care and support services to residents 65 years and older and to those of any age living with a physical disability who need help with every day activities.Our goal is to help participants remain independent, safe and comfortable in the place they call home, improve their quality of life and remain as active as possible for as long as they can.Working as a team, we ensure that you are receiving the services you need from RAVNAH or other community agencies. We will create an individual care plan to address your individual needs and goals. When needed, long-term care services are available 7 days a week.
A RAVNAH registered nurse or social worker will be your case manager who can assist you in gaining access to medical, social, financial and other supportive services, and will manage those services for you. Your Case Manager will meet with you wherever you are. We will do an initial assessment and determine the number of hours of care and services available to you and begin discussing financial paperwork
Even if you choose to hire a private caregiver, RAVNAH can still provide case management services through Choices for Care. We can even provide information and assistance to your caregivers to enable them to assist in your care.
Why choose RAVNAH?
When you choose a RAVNAH case manager, you have an entire health team at your disposal, including nurses, physical and occupational therapists, home health aides, social workers and nutritionist.
Why is this important?
As a Medicare certified home health care agency as well as a Choices for Care provider, RAVNAH offers you continuity of care. This means your RAVNAH Case Manager can quickly alert the home care team of any changes in your condition or circumstances before things get worse. By taking a pro-active stance, we can better manage your care, respond quickly to medical needs and help you remain independent for as long as possible.
Your RAVNAH Case Manager will visit you every 60 days and can be called at any time.
Help with things around the house may be just what you or a family member needs to live independently. Our homemakers can help maintain a safe, clean and healthy environment by assisting you with light housekeeping, laundry, meal preparation, shopping, errands and medication reminders.
In addition to the homemaker services above, our Personal Care Attendants can provide extended hours of service to help you with personal care such as bathing, grooming, toileting, transfers and ambulation.
To ease a person’s isolation, our caregivers will spend time with you, socializing, taking walks or going shopping.
Being able to get around is the key to independence for everyone. We provide transportation to medical appointments and other activities, including shopping, family and social events.
Respite for Caregivers
We provide assistance when families and other caregivers need a well-deserved break from the demands of care.
How do I qualify?
If you are eligible for Medicaid, Level 2 nursing home placement, meet financial criteria, and require help with daily living activities like bathing and grooming, you qualify for Choices for Care services.
