What is Home Health Care?

Typically, adult home care is appropriate when a person can safely stay at home but still needs ongoing care that cannot be effectively provided solely by family and friends. Patients recover quicker while maintaining their independence and dignity in the familiar, comfortable surroundings of home free from the stress and worries of caring for themselves or a loved one alone.

VNAHSR helps patients and families improve their health and teaches how to better manage their health care at home. Using the latest advances in medical technology, our compassionate team of highly trained nursing professionals, licensed therapists, social workers and home health aides can perform therapies and monitor patients in ways once reserved for hospital settings. As hospital stays decrease, more patients are recovering at home. With VNAHSR by your side, you will receive the highest quality of health care in the comfort and security of your own home.

We work with participants to keep them independent, safe and comfortable in the place they call home.

  • Choices for Care is a program that provides chronic care services for long-term Medicaid eligible clients who wish to remain independently in their own homes.
  • VNAHSR’s traumatic brain injury program provides in-home support for individuals recovering from brain injuries
  • Our homemaker program supports low-income elders and disabled adults who need assistance with meal preparation, housework, errands and companionship.

Medicare, Medicaid, and most private health insurance policies cover most home care and hospice services when ordered by a physician. As a non-profit home health agency, VNAHSR works to provide home health services to all who need them. VNAHSR can charge on a sliding scale for services not covered by insurance. We have nurses specially trained to discuss your options, answer questions, arrange services, and clarify insurance coverage. Our goal is to work with you, your physician and other health care providers to deliver coordinated care, regardless of the simplicity or complexity of your needs. We can often begin home care services within 24 hours of receiving a referral. If needed, a VNAHSR nurse will make a no-charge evaluation visit to determine if home health services are necessary and how they might be useful.
