What is CarePlus®?

CarePlus® is a private-pay in-home care program offering professional, personal care and companion services to help people of all ages take care of themselves or a loved one in the comfort of home. It is a program of the Visiting Nurse Association & Hospice of the Southwest Region – a Vermont home health agency your family, friends, and neighbors have trusted for generations.

Bed & Bath ServiceAssistance in the morning and at night with bathing, dressing, and personal grooming.
Check-In Service Monitor medications, food intake, daily weight.
Personal Care Monitor medications, food intake, daily weight.
Household Services Light housekeeping, shopping, meal preparation, changing bed linens and laundry.
Personal Companion Day or NightSupport to help maintain quality of life, including reading, sleeping, playing cards, games, writing letters, taking walks, going shopping.
Respite for Caregivers Tired during the day or trouble sleeping at night? Our caregivers will stay with your loved one, day or night, while you catch up on rest, errands, friendships or self-care.

Transportation and Errand Service Accompany you to medical or social appointments, including trips to the doctor, dentists, pharmacy, grocery store, hair salon or dry cleaners.
Hospital Recovery Service Coming home from the hospital can be a big adjustment. We can provide one-on-one support and assist with cooking, cleaning and shopping.
End of Life Care Support for individuals facing a life-limiting illness, along with respite for the caregiver, including light housekeeping and companionship.
New Mom Helper Assistance with baby care and housekeeping so Mom can spend more time with the baby or catch up on needed rest.
Telehealth A non-invasive, electronic, daily monitoring of vital signs and basic medical evaluation.

CarePlus not only provides you with non-medical care to help with your activities of daily living, we also offer medical care when you need it.

VNAHSR’s professional team of nurses and therapists provide training for our staff on an on-going basis so we are prepared to take care of unexpected health emergencies.

We understand how important it is to you and your family that you can trust the people we send to your home. You can have complete peace of mind knowing that your CarePlus companion is carefully screened, trained and closely supervised. We are committed to providing professional care with utmost integrity and care.

It begins with a phone call. We’ll set up a time for an experienced care coordinator to conduct a free in-home evaluation to determine what types of support and services you may need. Based on this initial assessment, a care plan is developed specifically for you, at no cost.

Once a care plan is accepted, we’ll carefully match a caregiver to fit your individualized needs. Then services are provided in a professional and compassionate manner.

Rates can vary depending on your individualized care plan. Please call our office for a personal in-home assessment. Services may be combined to reach minimum time requirements.

Services are typically paid for by clients or family members. Some long-term care insurance will pay up to 100% of the costs. We will work with you and your insurance provider to determine your level of reimbursement eligibility.
